Care Coordination (Local Systems of Care)

Provides case management services for families with children having serious emotional or behavioral problems. The philosophy is to support families in keeping their children safely at home and minimize out of home placement days with families as full participants in all aspects of planning. Child Specific Teams are held in Essex, Portland and Middletown. The teams advocate creating and obtaining services that allow children to remain in the community. The program is child centered, family-focused, and culturally competent.
