Continuum Management
Discharge Planning, also known as Continuum Management, assists in making the necessary arragements for patients who need special care after they go home from the hospital, and making referrals to a home care agency of the patient's choice. This service is offered only to patients being discharged from Middlesex Hospital and its satellite facilities.
