The Center for Chronic Care Management

The Center for Chronic Care Management consolidates Middlesex Hospital’s Disease Management programs. These unique, individualized programs assist patients with asthma, diabetes, chronic heart failure and tobacco addiction to achieve and sustain improved health.

The Center for Chronic Care Mangement is comprised of the following programs:

Little AIR – a pediatric asthma program especially tailored for the unique needs of children with asthma.

AIR Middlesex– an adult asthma care program.

Chronic Heart Failure Program– attends to patients in the hospital as well as those who have been discharged home with the diagnosis of heart failure.

Diabetes Programs – several components of care are available for diabetic patients including Medical Nutrition Therapy, Diabetes Education and one on one Diabetes Care Management.

Smoking Intervention Service – a smoking intervention counselor provides assistance to individuals and small groups to cope with their smoking addictions.

Eligibility: Patients must have a physician affiliated with Middlesex Hospital. If they do not already have a physician, the Center staff will assist patients in selecting one. All care managers work in cooperation with the patients’ physician to provide education and guidance to manage their chronic conditions. Patients with multiple diagnoses may be enrolled in more than one program.
